hereby solemnly affirm and sincerely state as follows:-
1. I am the petitioner herein
and the claimant in the main claim petition. I have filed the main claim
petition for claiming compensation on account of injuries sustained by me in a
motor vehicle accident.
2. In the above claim
petition this hon’ble court has passed an award of a sum of Rs.-------/ with
interest and cost on ----. Accordingly, the respondent has deposited a sum of
Rs. ------/- with interest and cost. Since I was minor, the said amount was
invested in the fixed deposit receipt in the bank till I attain majority.
3. At the time of filing the
main claim petition, I was minor. Now, I attained majority. My date of birth is
------. I am herewith producing my birth certificate to prove my date of birth.
My mother and guardian has also filed an affidavit about her consent to declare
me as major and my guardianship may be discharged. Otherwise, I will be put to
irreparable loss and great hardship.
It is therefore just and necessary that this hon’ble court may be pleased
to declare me as major and discharge my guardian from the guardianship and thus render justice .
