hereby solemnly affirm and sincerely state as follows:-
1. I am the petitioner herein
and the 1ST claimant in the main claim petition. We have filed the
main claim petition for claiming compensation on account of death of my husband
in a motor vehicle accident. The 2nd
claimant is my father-in-law, 3rd claimant is my mother-in-law, the
4th claimant is my sister-in-law, and 5th claimant is my
daughter.
2. In the above claim
petition this hon’ble court has passed an award of a sum of Rs.-------/ with
interest and cost on ----. The 1st claimant is entitled to Rs.----/,
the 2nd claimant is entitled to Rs.----/, the 3rd
claimant is entitled to Rs.---/, the 4th claimant is entitled to
Rs.----/ and the 5th claimant is entitled to Rs.----/.Accordingly,
the respondent has deposited a sum of Rs. ------/- with interest and cost. In
the said amount the 1st claimant share comes to Rs.---/ the 2nd
claimants share comes to Rs---/, the 3rd claimant’s share comes

3. While so, my father-in-law

justice, the petitioners 1,3 to 5 and proposed petitioner-NO 6 have to be
recorded as legal representative of the deceased 2nd claimant---.
Hence this petition.
It is therefore just and necessary that this hon’ble court may be pleased
to record petitioners 1,3 to 5 and proposed petitioner – NO 6 as legal
representative of the deceased 2nd claimant --- and thus render
justice.
